Extension Homemakers Club Mailing List Form

1. These instructions further support the information the Extension District Director has provided to the County Extension Agent - Family and Consumer Sciences on mailing lists.

2. This form, G-2 and G-3, is to be completed annually and signed by the President and Secretary of each Extension Homemakers Club in the county.

3. The names for this list should be copied from the Club Secretary’s Book to become a record of local club membership for the current year.

4. One copy of the completed forms G-2 and G-3 for each Extension Homemakers Club is due in the County Extension Office on, or before, June 1 of each year.

5. Each county should send Forms G-4, G-5, (EH County Council Officer & Leader) to the State AEHC Coordinator by August 1.

6. These lists becomes the official list of all County EH Club members and includes the Cooperative Extension Service faculty member who is the advisor. Any distribution of this list should be a joint agreement with the club and the advisor.

Name Tag Information

2. A note will be sent to the County Councils reminding them the orders are due by September 1 and February 1. Late orders will be held until the next ordering date. Only one order will be accepted from each county on each ordering date.

3. County Council Presidents will accept the names and money for the name tag orders from the Club Presidents. The County Council President will complete the form G-7 (on next page) in a “typed” or “printed” format. (AEHC is not responsible for misspellings) The County Council President will give three copies of G7 and one check to the AEHC District Director. The AEHC District Director will forward one check and three copies of the order to the State Treasurer. Council Presidents should keep a copy for their own records.

The order should include one check for the entire order made payable to the Arkansas Extension Homemakers Council. (Orders with more than one check will be returned.)
